For a noise report form, North Warwickshire’s published guidance doesn’t list a separate noise-specific form, but it does say an enforcement report needs key details to let officers investigate properly.

If you leave out enough information that officers can’t begin an initial investigation, the report can be rejected and you’ll be asked to resubmit it. You can submit a report through the online Report a Breach form, or send the same information by email to toplanningenforcement@northwarks.gov.uk.
