Perth & Kinross asks you to provide proof of identity and enough information to verify your housing circumstances before your application goes live. For some applicants, there are a few extra documents depending on your situation.

| Medical housing self-assessment form | If you or someone in your household has a health condition or disability and your current home is not suitable |
| Maternity certificate (MATB1) | If you are pregnant |
| Confirmation of sale of property | If your home is to be sold |
| Court summons for possession | If your home will be repossessed |
| References from a landlord | If you’ve been a tenant at any time in the past three years, unless you’re making a homelessness application |
| Letter from the person receiving or providing help | If you’re applying because you need to help someone live independently, or need help from someone to live independently |
If you don’t provide all the information needed, your application won’t go live until it’s complete. Perth & Kinross also says you can get help from the Housing Options Team to fill in the form.
