West Lothian asks you to apply online through the Admit Application Portal, and for school holidays applications you can also ask Pupil Placement for a form. For current school-year applications, the form is sent to the relevant school Head Teacher; for holiday applications, it goes to Pupil Placement and then on to the school at the start of term.

West Lothian also says to include any supporting evidence they ask for, such as proof of address, a birth certificate, or baptism certificate for Roman Catholic schools. If you want to keep your choice strongest, submit just one application and make sure it’s the most up to date, because only the latest application is considered.
